Denver, Colorado – May 24, 2019 – In 2015, The Colorado Behavioral Healthcare Council (CBHC) was awarded funding from the Colorado’s SIM Office to facilitate the selection of four CMHC sites to participate in a four-year program to integrate primary care and prevention services into the community behavioral health setting (bi-directional integration).
There are four bi-directional health homes that were selected through a highly competitive bid process to participate in SIM. They are: Community Reach Center’s Commerce City Clinic in Commerce City, Mental Health Partner’s Boulder Integrated Health Home in Boulder, Jefferson Center for Mental Health’s Jefferson Plaza Family Health Home in Lakewood, and Southeast Health Group, which has four SIM-funded health homes in La Junta, Rocky Ford, Las Animas, and Lamar.
