The Colorado Department of Health Care Policy and Financing is excited to announce the 2015 launch of the ACC Chronic Pain Disease Management Program modeled after Project ECHO programs. The program has been designed to improve primary care providers access to pain management specialists and to best practice treatment guidelines. Medicaid accepting primary care providers or practices can select the best element(s) from the Chronic Pain Disease Management program for their practice. The program will include the following three components:
- Chronic Pain ECHO-like program designed to give Colorado primary care providers greater access to pain specialists and improve usage of evidence-based best-practices with pain management clients.
- Buprenorphine ECHO-like program for licensed providers interested in gaining greater insights and experience treating opioid addiction with Buprenorphine/Suboxone.
- Practice Transformation Learning Collaborative for Pain Management & Addiction Issues will be an opportunity for interested teams of providers and clinic administrators to enroll in a practice transformation program focused on implementing pain management best-practices. This program will be modeled after nationally-endorsed quality improvement programs such as the Institute for Healthcare Improvement’s (IHI) Breakthrough Series Collaborative.
