Behavioral and Primary Healthcare Collaboration in Colorado

To demonstrate the commitment of the community mental health system in Colorado to integration, CBHC has undertaken a special project to map all of the locations where CBHC members are currently providing coordinated and integrated services.  This mapping project demonstrates the expanse of collaborative efforts underway and is a tool to help assess the different types of collaborative arrangements with different populations in Colorado.
